October’s ASC Coding Update Adds Ten Category III Codes and Billing Changes

CMS released its October ASC payment update on October 8, with changes effective October 1. Billing departments should reconcile the update against claims already submitted since the quarter began.

The agency is implementing 10 new Category III CPT codes. It is also deleting C8010 because CPT 1091T now describes the associated investigational carotid-filter procedure. Descriptors change for C1982, C9797 and C8004. These are specific coding changes, not an across-the-board expansion of coverage for every emerging procedure.

The drug section is substantial: 12 new HCPCS codes with pass-through status, 10 code deletions and payment-status changes affecting 21 codes effective October 1. Another payment-status change is retroactive to July 1. CMS also identifies three drug descriptor changes, eight new skin-substitute codes and five skin-substitute descriptor revisions retroactive to April 1.

The operational task is to connect each relevant change to the center’s inventory, charge master and claims system. A code can exist in the software while its units, descriptor or payment indicator remain wrong.
Administrators should request a focused review of affected services rather than assume that a software update completed the work. Compare the product purchased, the dose administered, the units billed and the applicable payment file.

CMS explicitly distinguishes assigning a code and payment rate from making a coverage determination. That matters when a vendor describes a new code as evidence that Medicare will pay for its product.

For this quarter, ask the billing manager to identify applicable changes, affected October claims and any earlier claims requiring review. Keep the supporting documentation with the correction. A quarterly coding update becomes useful only when the appropriate changes reach the people entering charges and submitting claims for payment. Assign responsibility for checking that those corrections stick.
