Prior authorization may be the only part of medicine capable of raising a surgeon’s blood pressure before the patient even enters the building.

CMS’s proposed 2027 OPPS/ASC rule would expand Medicare’s hospital outpatient prior-authorization program to include additional botulinum toxin injection services. The proposal is part of a broader federal effort to control unnecessary utilization in outpatient settings.

This deserves attention from pain physicians, neurologists, physiatrists and facilities where botulinum toxin is used therapeutically. Prior authorization changes the economics of a service even when the reimbursement itself does not change. Every authorization requires staff time. Every delay creates an opportunity for a cancellation. Every denial creates an appeal. And every expensive drug ordered for a case that subsequently disappears creates inventory risk.

The business lesson is larger than botulinum toxin.

When CMS places a service under prior authorization, the front office becomes part of the clinical production line. A center can have an empty OR at 10:00 not because the surgeon was unavailable, but because a document was missing at 10:00 two weeks earlier.
High-performing centers increasingly treat authorization like revenue-cycle surgery. Identify the indication. Verify coverage. Obtain the clinical documentation. Submit early. Track the payer response. Escalate before the scheduled date. Confirm again before purchasing expensive supplies or medications.

This also means that physicians must understand the documentation requirements supporting the procedure. A vague note that may be clinically understandable can become financially disastrous when reviewed by someone who has never met the patient.

CMS’s proposal is not yet final, so facilities should not change billing protocols based solely on the proposed rule. But the direction is worth watching.
